This section is for documents required to help you manage your education and treatment plan.
We do not write reports for court.
We recommend providing as much notice as possible to minimise deadline stress for you.
VCAA and VTAC forms
These forms are quite detailed and time-consuming. As they require input from and discussion with the patient, these can be completed within a session. When completed outside of a session, however, the fee does not attract a medicare rebate.
GP Letters
We strive to maintain effective communication with your GP. Mostly this includes letters about diagnosis and treatment plan once this is clear, usually after the assessment period, and then when the treatment is altered.

School and University Letters
Additional documentation for School and University and other stakeholders can be attended to within sessions. Where these documents are requested outside of sessions, the cost depends on the amount of time required to write the document, and will not attract Medicare rebate.
Medical Certificates
Patients need to speak to Dr Alexander on the day a medical certificate is required. Backdated certificates are not provided.
